What sets this apart from generic cylinder sleeves?
The bottle shape has a reduced diameter frenulum area to really hold off that donut formation and an angled section to seat behind the glans to press that upper frenulum where the donut likes to pop. I added reinforcement to the frenulum area since it reshapes quite a bit when your girth expands towards the end of the session. This helps hold pressure on your frenulum to keep the edema from creeping up and out of the sleeve. I've found I can double my session time before a donut ends up forming.

How to Use
The goal is to have wrap the sleeve inlet over the cylinder flange to hold it in place during pumping.  The sleeve inside the cylinder should be pressing against your ridge with the glans out of the sleeve for this to properly restrict edema.  
Size the sleeve length needed to seat the sleeve cut out to your coronal ridge
Hold up the sleeve next to your member, use your fingers to hold the length required
As gently as possible, stretch the sleeve over the cylinder flange wrapping the excess sleeve up the outside of the cylinder
This works best before adding any lube
Apply lube to the inside of the sleeve and enter the sleeve while pulling vacuum
Readjust the sleeve length as needed
You may have to readjust slightly during a pump session due to size changes
 
Usage Tips 
Start with a sleeve length that somewhat pushes against your ridge as your glans should grow over the course of a session.  This minimizes the need to readjust the sleeve length
It's easier to start with a shorter amount of sleeve inside the cylinder and feed sleeve into it
This is just from lube making it a little more difficult to hold the sleeve
Just wipe off any lube before adjusting to avoid this difficulty
When coming out of the cylinder at the end of a session, you can twist the sleeve to help get the glans back into the sleeve or you can remove the sleeve from the cylinder flange first, take off the cylinder, then remove the sleeve.
 
Lubricant and Cleaning Tips
Avoid silicone and petroleum based lubes and products such as petroleum jelly (vaseline) and mineral oil (found in many lotions)
Water based lube is suggested but high quality hybrid water-silicone lubes may be acceptable
Natural oils are acceptable for use but the friction is higher with these oils than with water based lubricants
